STEP 1 -

Complete Form 437A Request Approval For Intra State or Out Of State Student Travel Form. Turn in to Principal.

STEP 2 -

When Field Trip and Activity is approved. Print out the Parent/Legal Guardian Authorization for Student Participation and Travel Form for each student participating.

Note: Inform Cafeteria Manager of how many students will be participating if activity pass the lunch for middle school and/or high school.
