You can scan the QR code on the event flier to fill out the forms online, or contact the school nurse to request paper copies.
🩺 What is Hawaiʻi Keiki?
The Hawaiʻi Keiki program works in partnership with the Hawaiʻi Department of Education to keep students healthy and ready to learn. By signing and submitting the Hawaiʻi Keiki Consent Form, your child can receive no-cost healthcare services on campus, including:

First aid and emergency medical response

Management and care for chronic conditions (allergies, asthma, etc.)

Administration of over-the-counter medications (e.g., Tylenol, ibuprofen, albuterol) when needed (parents are contacted beforehand whenever possible)

Connections to a Nurse Practitioner specializing in mental health and social-emotional support
-
Health education and counseling
Note: Hawaiʻi Keiki services are free and do not replace your child's primary care provider.
