Attention all student-athletes, parents and guardians, Winter sports tryouts are right around the corner, and spring sports aren't far behind! Before our athletes can hit the field, court, track, or pool, Hawaiʻi public school rules require a completed annual physical exam.


To make sure every athlete is cleared and ready to compete for the upcoming winter and spring seasons, Hawaiʻi Keiki is offering no-cost sports physicals directly on campus for students who are unable to see their primary care provider!

  • Date: Friday, October 2, 2026

  • Time: 8:00 AM – 12:00 PM

  • Location: Pāhoa High and Intermediate School, HK Room

  • Requirement: By appointment only. No walk-ins will be accepted, so lock in your spot early!

  • Deadline: Registration closes 2 business days before the event date.

  3. HIDOE Physical Examination for Athletes Form
