Hawai'i Keiki is offering no-cost physical exams for public school student athletes who are unable to see their Primary Care Provider (PCP). An annual physical is required for students to participate in sports.

The exams will be held on Tuesday, January 20th, 2026 , at the Hawai'i Keiki Office at Pāhoa High and Intermediate School. Appointments are required, and there will be no walk-ins. Registration must be completed at least two business days before the event.

Students will need parent permission to be able to take advantage of this opportunity.

To confirm their appointment, students must complete the following three forms by scanning the provided QR code:

  • Hawai'i Keiki Registration Form

  • Hawai'i Keiki Consent Form

  • HIDOE Physical Examination for Athletes Form
